Preventing a miscarriage is not always possible, but in many cases the risk can be reduced through a targeted, individual approach. The Dutch Society for Obstetrics and Gynaecology advises, in cases of recurrent miscarriage, to carry out further investigations into possible underlying causes, such as hormonal, genetic, anatomical, and clotting factors.
Within the FertilityCare approach, the emphasis is on optimising natural fertility before conception. The menstrual cycle is seen as the primary starting point for this. Through systematic cycle monitoring, hormonal patterns are mapped out in detail, including follicle maturation, the timing and quality of ovulation, and hormonal balance in the luteal phase. Deviations such as suboptimal ovulation or a progesterone deficiency can thus be recognised early on.
In addition to hormonal factors, possible disruptions in the vaginal and endometrial microbiome are also being investigated. When there are indications of immunological imbalance, both in the reproductive organs and systemically, these are also treated where necessary, with the aim of improving the immune environment.
Based on the insights obtained, a personalised treatment plan will be drawn up. This may include hormonal support, ovulation induction or optimisation of the luteal phase. In addition, depending on the situation, further investigation will be carried out into thyroid function, metabolic health (such as insulin resistance), coagulation, the immune system and the microbiome. The aim is to make the hormonal and biological conditions for a healthy conception as optimal as possible.
Therapy during pregnancy
Following a positive pregnancy test, care focuses on supporting a stable pregnancy, particularly in the early stages when implantation and placenta formation occur.
Scientific research and clinical observations indicate that physiological levels of oestrogen and progesterone are associated with a more favourable pregnancy outcome. These hormonal reference values form the basis for considering potential supplementation. In situations where support is indicated, the duration and dosage of hormone supplementation are determined individually and tailored to the clinical course and the patient's needs.
The support also includes regular check-ups, including ultrasound and laboratory tests, to monitor the progress of the pregnancy and to make timely adjustments if necessary.
The approach is always individual and based on the patient's history, previous pregnancy outcomes, and the progression of the current pregnancy, with the aim of supporting the most stable and optimal pregnancy possible.
