How do I book a first appointment with the doctor?

Carefully follow these steps to prepare for your first consultation. This will ensure the appointment is efficient and you are medically well-prepared.

Step 1 — Request a referral from your GP If you wish to be considered for reimbursement by your health insurer, you will need a referral from your GP beforehand. Therefore, arrange this before you schedule an appointment.

The FertilityCare Clinic in Kleve falls under planned, non-emergency care abroad at a non-contracted hospital. The following conditions generally apply for reimbursement by a Dutch health insurer:

• A valid referral from a Dutch GP, requested before the start of care
• Reimbursement depends on your policy type (reimbursement or in-kind policy) and your remaining excess.

Step 3 — Fill in the intake forms and bring them with you Before your consultation, you will receive intake forms by email. Please complete these in advance and bring them with you to your appointment. Please also bring relevant medical information, such as previous fertility test results or outcomes from prior treatments. This will help the doctor form a complete picture of your situation.
